According to Shiloh's name tag he/she is property of some Medical Center, therefore a trained service support animal. Of course dogs don't need money. They just need owners who have enough money to provide food/shelter/training. So hopefully Shiloh is generating some method of income to contribute to its own upkeep.

Oh especially in healthcare they love to not pay critical roles well.
Everyone knows nurses aren’t paid nearly well enough for the work they do. But what less people know less about, or care less about, is how little EVS (environmental services) techs get paid. They get paid literally below poverty level wages (ask me how I know), despite it literally being an extremely important role in the healthcare system. Cause I doubt anyone wants to go a dirty hospital when you need care.
